Event Waiver

I give full consent and authority for the Player to attend the session and I accept full responsibility for them.

By participating in the activity (and except to the extent that the same may be precluded by law) I agree to release and indemnify The Field Hockey Academy and its Operators, Owners, Staff, Coaches and Contractors and all others related to The Field Hockey Academy operation as follows;

1. The Player participates in the activities at the venue at their own risk and responsibility.
2. I release and hold harmless The Field Hockey Academy, its employees and agents, from and against all and any actions which may be made by me or on my behalf or by other parties for or in respect of, or arising out of injury, loss, damage or death caused by or to the Player, the Players or my property whether by negligence, breach of contract or in any way whatsoever.
3. I accept all responsibility for any injury, loss or illness that may arise from the Player’s participation in The Field Hockey Academy events.
4. I also agree that in the event that I or any children in my custody or on the premises of the event are injured or my property is damaged, I will bring no legal claim or otherwise against The Field Hockey Academy, its employees and agents in respect of that injury or damage.
5. I grant permission to The Field Hockey Academy and it’s partners and affiliates, for the rights of my image, likeness and sound of my voice as recorded digitally without payment or any other consideration for content that is captured at or around the The Field Hockey Academy events.  I waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.  I also understand that this material may be used in diverse educational settings within an unrestricted geographic area. I grant permission for the photographic, audio or video content to be utilised by The Field Hockey Academy and it’s approved partners. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.
